Dear Parents,

 

Beginning with the 2003/2004 school year, the state requires that all sixth graders shall provide documentation of varicella (chicken pox) immunity.  Students will not be allowed to enter the sixth grade without documentation of varicella.

 

 

The documentation of varicella immunity shall consist of one of the following:

 

1)      Documentation of immunization against varicella.  Documentation of varicella immunization shall include the month, day, and year of administration.

a.       Children less than 13 years of age shall provide documentation of having received one dose of vaccine on or after 12 months of age.

b.      Children who are vaccinated on or after 13 years of age shall have received two doses of vaccine separated by at least one month.

2)      A physician’s statement attesting to a history of clinical varicella. 

3)      A parent or guardian’s statement attesting to a history of varicella.

4)      Documentation of immunity by confirming laboratory test results

 

Please provide this documentation to the health office prior to your child entering the sixth grade.
