Cooperative Middle School
Immunization Requirements
Parents of current 5th graders will receive paperwork in
May from your current school nurse pertaining to immunization
requirements prior to your child entering the 6th grade.

School Screening
The nurse’s office will start the health screening of all 6th graders
each September. This screening will include: Height, Weight, Vision,
Hearing, and Blood Pressure. We will also screen the 8th graders. The
screening on the 8th graders will also include Scoliosis screening. If
we find a problem or concern, we will send a referral sheet home with
your child and follow up with a telephone call in several months. We do
not normally screen the 7th graders unless a need is determined.
Health Insurance
It has come to our attention that all students are now required to
have health insurance in order for them to attend a field trip. It
appears that not all our students have insurance, and for this reason we
have collected information on various health insurance agencies and
policies that may be helpful to you:

We also have available a student insurance plan. This plan can
cover the school day or a 24-hour coverage plan. Please contact the
nurse’s office for further information.

School has begun and so have the colds and flu symptoms! Our goal is
to keep our students as healthy as possible. A great emphasis is placed
on the students to wash their hands before eating and after using the
bathroom. Although it would be impossible to keeps all colds out of our
school, reinforcing handwashing at home would be a tremendous help.
I am also looking for spare socks, {they don’t have to match} unisex
colored sweat pants size 6-10, mittens, and snow pants. The weather will
be changing again sooner than we expect it to, so when you are doing
your fall cleaning, keep the health office in mind.
