Immunization Requirements

Ed 311.01 School Immunization Program.

(a) A parent or legal guardian shall have a child immunized against certain diseases including, but not be limited to, diphtheria, mumps, pertussis, poliomyelitis, rubella, rubella, and tetanus as well as other diseases which require immunization as determined in rules adopted by the Director of the Division of Public Health Services.

(b) A child shall be exempt from immunization in accordance with RSA 141-C:20-c.

(c) A child who is exempt from immunization in accordance with (b) shall not attend school during an outbreak of a communicable disease for which immunization is required under RSA 141-C:20-a.

(d) If a child is not immunized as in Ed 311.01(a), does not have a religious or medical exemption, or is unable to provide evidence of acceptable immunization for conditional enrollment, he/she shall not be admitted to school.

Immunizations

New Hampshire School Immunization Requirements [K/1st, whichever is entry level in your schools]

Students must present an official immunization record on the first day of school. If they require more than one dose of vaccine, they must present those doses already administered and an appointment card for the remaining dose(s). Students may also present laboratory evidence of immunity.

DTP/DT/DTaP/Td:

< 7 Years of Age Four Doses, 4th dose on or after the 4th birthday, or Five Doses
 > 7 Years of Age Three Doses, 3rd dose on or after the 4th birthday
10 Years since last Td One Dose of a tetanus-toxoid containing vaccine
(Td is the preferred vaccine.)
Acceptable intervals: * Dose 1 shall have been administered at no less than 6 weeks of age
* Doses 2 and 3 shall be separated from the previous dose by a minimum of 4 weeks.
* Dose 4 shall be separated by a minimum of 6 months from dose 3.
* Dose 5 is recommended at 4 – 6 years of age and shall be separated by a minimum of 6 months from dose 4.

POLIO:

K - 12 Three Doses, with the last dose after the fourth birthday of an all eIPV or all OPV schedule; or Four Doses of any combination of eIPV and OPV regardless of age at administration. (When a combination of vaccines have been administered, 4 doses are necessary even if the 3rd dose was administered after the 4th birthday.)
Acceptable intervals: * Dose 1 shall have been administered at no less than 6 weeks of age.
* All subsequent doses shall be separated by a minimum of 4 weeks.

MEASLES:

Entry (K or 1st) Two Doses of measles-containing vaccine.
7th – 12th Grade Two Doses of measles-containing vaccine.
Acceptable intervals: * Dose 1 on or after 12 months.
* Dose 2 at least 28 days after the first dose.

RUBELLA and MUMPS:

K - 12 One Dose of rubella and mumps-containing vaccine administered on or after age 12 months.

HEPATITIS B VACCINE:

Born on or after 1/1/93 Three Doses.
Acceptable intervals: * Doses 1 and 2 separated by at least 28 days.
* Dose 3 separated by a minimum of 4 months from dose 1, and 2 months from dose 2, and administered on or after 6 months of age.

VARICELLA VACCINE:

K-12 1 Dose or parent record of the disease.

For all minimum intervals and age requirements a 4-day grace period is acceptable.

The vaccines and doses above are the minimum requirements for school attendance.
The “Recommended Childhood Immunization Schedule, United States 2002” should be followed for the doses necessary for good preventive health.

Exemptions

Under RSA 141-C:20-c, the following exemptions are allowed:

Medical - A New Hampshire physician may submit, on their letterhead, that immunization against a particular disease may be detrimental to the child's health. An exemption for one particular disease will not affect other required immunizations.

Religious - A parent may object to immunization because of religious beliefs. They must sign a notarized form supplied by their school.

Cooperative Middle School Immunization Checks:

  • all sixth graders shall provide documentation of varicella (chicken pox) immunity. Students will not be allowed to enter the sixth grade without documentation of varicella.  Sample letter containing details.
  • before entering the seventh grade, your child will require his/her second MMR (measles, mumps and rubella) immunization. Students will not be allowed to enter the seventh grade without documentation of MMR (measles, mumps, and rubella).  Sample letter containing details.
