Immunization Requirements

PRE-SCHOOL CHILDREN 3-5 YEARS OF AGE

All minimum intervals and age requirements as stated in School Requirements Outline.

DTP/DT/DtaP:

  • Four doses, if it has been 6 months from Dose 3.
  • Do not need 5th dose after age 4 years, until primary school entry.

POLIO:

  • Three doses.
  • Do not need one after age 4 years, until primary school entry.

MEASLES, MUMPS and RUBELLA:

  • One dose administered on or after age 12 months.

HAEMOPHILUS INFLUENZAE TYPE B (HIB):

  • One dose after 15 months of age; or 4-dose series with the last dose being administered at 12 months of age or greater.
  • HIB is not required for children over 5 years of age.
  • If the products PedVax HIB or Comvax have been used, 3 doses, with one after 12 months of age, is acceptable.

Acceptable intervals:

  • Dose 1 shall have been administered at no less than 6 weeks of age.
  • All subsequent doses shall be separated by a minimum of 28 days, provided the last dose was administered after 12 months of age, and separated by a minimum of 2 months from the previous dose.

HEPATITIS B VACCINE:

  • Three doses.

The 4-day grace period for minimum intervals and ages applies to the above requirements.

Exemptions - Under RSA 141-C:20-c, the following exemptions are allowed:

Medical - A New Hampshire physician may submit, on their letterhead, that immunization against a particular disease may be detrimental to the child's health. An exemption for one particular disease will not affect other required immunizations.

Religious - A parent may object to immunization because of religious beliefs. They must sign a notarized form supplied by their school.
